Terbinafine hydrochloride: 250mg and 1% Cream


Introduction: Terbinafine hydrochloride is a synthetic allylamine antifungal. It is highly lipophilic in nature and tends toaccumulate in skin, nails, and fatty tissues Antifungal, allylamine -one of the first antifungals of the allylamine class, discovered in1974. It was approved for systemic use in the UK in 1991, and for topical use in the USA in 1992.

Terbinafine is an antifungal effective against Dermatophytes, Aspergillus sp., and Candida and Pityrosporum yeasts. Terbinafine isonly fungistatic against Candida, however cure is possible after 2-4 weeks of treatment (-).It is available in systemic and topical formulations. Systemic treatment is usually reserved for infections of the nails, extensivecutaneous infections or those, which have not responded to topical therapy.

Mechanism of action: Terbinafine interferes with fungal ergosterol synthesis by inhibiting the enzyme squaleneepoxidase inthe fungal cell membrane.

This leads to fungal cell death by disruption of fungal membrane function and cell wall synthesis. The oral dose is well absorbedfrom the gastrointestinal tract, and is extensively distributed to the hair follicle, nail plate and sebum-rich skin.

Pregnancy: Terbinafine is considered a pregnancy Category B medication.Using of the drug during pregnancy is notrecommended, since the full risks are not known and the treatment of nail fungus can be postponed until after pregnancy.

Nursing mothers: Terbinafine is passed through breast milk. Lamifin should not be initiated during breastfeeding.

Contraindication: If having hypersensitivity to terbinafine hydrochloride

Side effects: Cases of taste disturbance, including taste loss, have been reported; Depressive symptoms independent of

taste disturbance have been reported. Other adverse reactions which have been reported include malaise, fatigue, vomiting,

arthralgia, myalgia, acute pancreatitis, reduced visual acuity, visual field defect, and hair loss.

Dosage & Administration:

Cream:Twice Daily

Wash your hands before and after use. If treating hand or nail infections, wash hands before useonly. Apply enough products to cover the affected skin or nail and surrounding area. Do not cover the treated area with a bandage ordressing unless your doctor or health care professional tells you to. Do not get this medicine in your eyes. If you do, rinse out with

plenty of cool tap water. Use this medicine at regular intervals. Do not use more often than directed. Finish the full course prescribedeven if you think your are better. Do not skip doses or stop using this medicine early.


Tablets:Once Daily


Less than 12 kg of body weight not recommended andless than 20 kg 62.5 mg once daily,

20-40 kg of body weight 125mg once daily and more than

40 kg and above 250 mg once daily

Mechanism of Action:

Terbinafine inhibits ergosterol synthesis by inhibiting squaleneepoxidase, an enzyme that is part of the fungal cell membrane synthesis pathway. Terbinafine prevents conversion of squalene to lanosterol, ergosterol cannot be synthesized. This is thought to change cell membrane permeability, causing fungal cell lysis.

Indication & Application:

·       Tineapedis (Athlete’s foot)

·       TineaCruris

·       Fungal nappy rash

·       Pityriasis

·       Preferable for intertriginous area’s associated with fungal infection associated

The powder can be applied once or twice daily. However, the duration of treatment varies according to the indication & severity of infections.

Adverse Effect: 

Allergic skin rash including urticaria and erythema.

Storage Condition:

Store below 250 C & to protect from light.
